Decentralized Health Network

The Decentralized Health Network is our mission to connect India's most under-served regions with essential healthcare services by 2036. Unlike centralized models that rely on urban hospitals, this network empowers local communities through Smart Healthcare Machines, Tele-Diagnostics, and real-time consultations - right at the village level. It's a scalable, tech-driven approach that ensures no region is left behind, bridging the healthcare gap across 600,000+ villages with speed, accuracy, and accessibility.

Network Workflow of Nirmaan Health Initiative

A scalable network of Tele-Consultation & Tele-Diagnostic Centres (TCTDs) and Mobile Tele-Clinic Units (MTCUs).

Tele-Consultation & Tele-Diagnostic Centre

  • Installed at district sub-divisions, serving as the main centre for healthcare services.

  • Staffed with specialist doctors (via Telemedicine), nurses and technicians.

  • Equipped with Smart Healthcare Machine, ECG facility, advanced diagnostic tools and EHR.

Mobile Tele-Clinic Unit

  • Each TCTD is supported by 1 MTCUs per 300 sq. km area.   

  • MTCUs travel daily to remote villages, providing on-site health screenings, diagnostics and tele-consultations.

  • Each MTCU has 2 nurses and a driver, ensuring full medical assistance on the ground.

Seamless Referral & Follow-up System

  • Patients needing further treatment are referred to the nearest TCTD or associated hospitals.

  • AI-powered patient records ensure continuous monitoring and follow-ups at both MTCUs and TCTDs.

Tele-Diagnostic Workflow

A connected care journey - from instant health screening to real-time consultations and seamless referrals to specialists.
